The teachers of a four-year-old who died after becoming trapped between two stair gates will be called to give evidence at a second inquest into the tot’s death.

Draco Chapman died on April 23, 2019, after suffering "irreversible and catastrophic brain injuries" following the tragedy at his home in Lakenham, Norwich, five years ago. 

His teenage brother called 999 after discovering Draco trapped between two stair gates put on top of each other between the living room and hallway on April 2.

Draco was taken to the Norfolk and Norwich Hospital and then Addenbrooke's Hospital where he was put into an induced coma. 

His ventilation was removed on April 18 and he died at the Quidenham Children's Hospice, run by East Anglia's Children's Hospices, days later.

An initial inquest into his death concluded that it was an accidental death, but the original finding was later overturned

Shortly after the original conclusion, a serious case review was published by Norfolk Safeguarding Children Partnership. 

The report stated that concerns were raised by teachers and health visitors in the months before Draco died. 

It also made a recommendation that a safety warning should be put on stair gates. 

During a pre-inquest hearing held on Tuesday, senior coroner Jacqueline Lake confirmed that staff from Tuckswood Academy and Nursery would be called to give evidence. 

While a date for the second inquest has yet to be set, it has been scheduled to take place over five days at Norfolk Coroner’s Court, based at County Hall.

Speaking after his son’s death, Draco's father David Chapman said that his family had been "destroyed" by the youngster’s traumatic death.  

Paying tribute, he added: "Draco was a very mischievous little boy. The whole family loved him dearly.”
